I hereby voluntarily release, waive, and discharge Texas Supremacy of Music and Arts Conservatory (TSMAC), its directors, officers, employees, volunteers, partners, sponsors, and affiliates from any and all liability, claims, demands, or causes of action arising from injury, illness, or damages sustained by my child during participation in this event. I understand and agree that TSMAC is not responsible for injuries, accidents, or loss of personal property during the event. *
